Case Manager - Medical – Intermediate Professional (PSU2128)
Job Summary
The Case Manager - Medical collaborates with partners across the University to ensure that basic, medical, academic, and psychosocial needs are addressed for all university patients; provides resources to University stakeholders on case management models; promotes health equity by offering additional case management services for patients in need of financial, health, and mental health resources, including accessing and applying for any assistance programs; monitors daily referrals and reviews incident reports; keeps detailed records and documentation to ensure use of case management database is utilized efficiently; assesses client’s needs and refers to other departments or entities when appropriate; may provide outreach to families when requested by patient.
Representative Duties
- Serve as a clinical liaison for university students, faculty, and staff, and provide evaluations, preliminary mental health assessments, and behavioral health care referrals
- Manage clinical caseloads; may need to provide follow-up assessments in specific cases
- Gather sufficient information from all relevant sources to determine the effectiveness of the case management plan; modify the plan as necessary utilizing input from client, physicians, and other appropriate sources
- Make referrals to community therapists and psychiatrists when necessary and available; as needed may schedule referrals, testing, therapy, etc.
- Consult with medical staff to best provide patient care when needed
- Research and connect patients to a variety of resources including applications, reading materials, and coping mechanisms
- Maintain knowledge base of local/external resources and coordinate with outside health providers, treatment centers, and community services
- Support patients in the navigation of medical bills and healthcare costs
- Be a resource to patients to help understand insurance programs or apply for qualifying insurance

Salary Structure
- Comp Grade – 9
- Minimum – $56,200.00
- MidPoint – $68,800.00
- Maximum – $81,500.00
